Our goal at Primary Care Plus is to improve your healthcare and provide you access to your health information and your healthcare provider in an easy and convenient manner. We The Primary Care Plus Patient Portal is a HIPAA-compliant, password-protected, and secure section of our site that is designed with your privacy in mind. The patient portal supports your ability to request a prescription refill, update demographics, add or update pharmacy information, and communicate with the doctor and staff.
The Patient Portal is NOT for Emergencies. Please call the office directly at (678)574-3000 or dial 911 if you think you are having a life-threatening emergency.
Logging into the Patient Portal
When we enroll you as a patient, we will give you a User Name and Temporary password. Please maintain the temporary password until you create a permanent password. Your login credentials guarantee that your communications with our staff will be secure and confidential.
Click the Primary Care Plus patient Portal link here. The patient portal login page will display in a new browser window.
Login using the user name and temporary password assigned by the office. Please maintain the temporary password until you create a new password. Once you enter the new password read the Patient Portal Agreement and Privacy Policy
Check the Box to Agree or Disagree. If you agree you will be taken to the page to Complete Forms. Click on Complete Forms.
Complete the Problems sections . Click Next
Fill out Patient History.
Click Update History
Click Next: Drug Allergy Complete this section
Click Next: Medical Surgeries Complete this section
Click Next: Main Social Section Complete this sections
Click Next: Protected Health Information
Click Next: Submit Forms
